Records Requests - As part of the concierge service we will request medical records on your behalf.
There is no additional charge for this service; however, any charges incurred from the doctor or facility will be charged back to the subscriber. |

** Cancellation policy – Any subscription may be cancelled at any time by the subscriber.
Subscriptions will be subject to a $25 cancellation fee if the service is terminated prior to the completion of the first 12 month period.
